Senior Social Worker

Permanent Part Time opportunity to deliver Social Work services to consumers within the multidisciplinary residential transition care program, applying high level skills in assessment and intervention to promote, maintain and improve their health, functional ability and quality of life.
About this role
Permanent Part-time opportunity to develop a low intensity restorative program which considers personal choice, needs and lifestyle. Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Deliver high level Social Work clinical services within the residential transition care multidisciplinary team in accordance with prescribed professional and ethical standards, including comprehensive psychosocial assessments and interventions, coordination of care and discharge planning.
- Utilise high level communication and interpersonal skills to achieve optimal consumer and service delivery outcomes.
