Nurse Practitioner - Diabetes Service
<
Metro North Health is seeking an experienced Nurse Practitioner to deliver advanced, specialist nursing care within the Diabetes Service, supporting both paediatric and adult consumers. This role operates at an advanced scope of practice and plays a key leadership role in delivering innovative, nurse‑led, recovery‑oriented models of care.

About the role
Working collaboratively within an interprofessional team, you will directly contribute to improved clinical outcomes, service development, and consumer self‑management of diabetes using contemporary and technology‑enabled approaches
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Demonstrate autonomy, accountability, and competence for expanded practice in providing client-focused care within a nursing model including advanced health assessment of consumer's/patient's clinical condition, development, and implementation of appropriately documented management plan.
- Evaluate service outcomes with specific indicators and benchmarking and apply appropriate change management strategies as required.
- Demonstrate the ability to promote, undertake and support research activities to support improved client care and best practice.
