Permanent part-time opportunity to provide personal care assistance and support services to eligible clients requiring short-term, time-limited intervention following hospital discharge or Emergency Department presentation.
Post Acute Care Service (PACS) delivers multidisciplinary, short-term care to support clients' recovery in home or clinic settings and prevent hospital readmission. Services operate 7 days per week, including public holidays, with a focus on responsive, client-centred care.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Demonstrated ability to work independently and with minimal supervision whilst seeking direction/guidance from service coordinator and clinicians
- Demonstrate effective interpersonal and communication skills including tolerance and respect to the lifestyle and standards of clients
- Ensure confidentiality in relation to client, carers and colleagues
- Perform a arrange of tasks that include travel and providing personal cares to clients in their homes
