Your opportunity:
- The Team Leader/ NUM is a clinical leader, accountable at an advanced practice level for the coordination of patient-centred clinical practice and the provision of human and material resources within the Integrated Care Rehabilitation Services. This role will lead a multi-disciplinary team in the delivery of hospital avoidance strategies, chronic condition pathways and innovative hospital in the home initiatives which aim to improve the health and wellbeing of Integrated Care Rehabilitation Services clients by delivering the right care in the right place at the right time.

Our offer:
You will receive a competitive remuneration package value up to $173,368 p.a., comprising salary between $139,951 - $151,949 p.a. (Nurse Grade 7) or $144,991 - $151,289 p.a. (HP5), employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (Nurse Grade 7/HP5) (Permanent Full Time, 76 hours p.f.)
