About this role
Your new role:
As a Senior Social Worker within the Pathways Team, you will play a critical role in supporting complex long-stay patients and driving timely, safe discharge outcomes across Metro South Health.
- Deliver high-quality, evidence-based social work services in acute and subacute settings
- Lead complex case management and discharge planning for long-stay inpatients
- Collaborate across multidisciplinary teams to remove barriers to patient flow and care
- Contribute to service development, quality improvement, and clinical education initiatives
About you:
You are a skilled and compassionate social work professional who thrives in complex clinical environments and is committed to improving patient outcomes.
- Demonstrated expertise in complex discharge planning and long-stay patient management
- Strong communication and collaboration skills, with the ability to influence stakeholders
- Proven ability to prioritise and manage a demanding caseload with sound clinical judgement
- Experience providing supervision, training, and contributing to service improvement initiatives
