Your new role:
As the Clinical Nurse within the General Medicine Patient Flow Initiative, you will play a key leadership role in delivering advanced clinical care and improving the patient journey across a busy and dynamic healthcare environment. Working collaboratively with multidisciplinary teams, you will support timely discharges, coordinate complex care, and help drive innovative patient flow strategies that enhance safety, efficiency, and patient outcomes.
In this role, you will:
- Deliver advanced clinical nursing care for patients with complex needs
- Lead and coordinate patient flow and discharge planning activities
- Support implementation of Criteria Led Discharge (CLD) practices
- Provide clinical leadership, mentorship, and support to nursing staff
- Collaborate with multidisciplinary teams to reduce operational delays

You are an experienced and motivated Clinical Nurse who thrives in a fast-paced healthcare environment and is passionate about improving patient outcomes. You bring strong clinical leadership skills, excellent communication, and the ability to confidently manage complex situations while supporting team success.

About us:
The department of Internal Medicine and Clinical Epidemiology provides general medicine inpatient care to approximately 110 patients who are mostly of older age, with multiple co-morbidities and with various impairments. It has approximately 5000 admissions and over 1500 outpatient visits per year. The department comprises 5 general medicine consultant units and a consultant-led perioperative medicine service, and interfaces with the Medical Assessment and Planning Unit, the Hyperacute Stroke Unit, and clinical pharmacology, palliative care, geriatric rehabilitation departments, and Hospital in the Home programs. It has well-established teaching and research activities for medical students, junior staff and physician trainees.
