RaSS Administration Officer

The RaSS offers support to the 16 Residential Aged Care Homes (RACH) located within the West Moreton Region, with a population of approximately 1400 residents
Approximately 100 RACH residents present to the ED each month. These patients have a higher likelihood of inpatient admission. Since the RASS service commenced, there has been a slow reduction in the number of RACH residents presenting to the ED
The service operates between 0800-1830, 7 days a week and is based on Level 1 of the Hayden Centre.
Permanent Part Time position – 38 hours per fortnight.

About the role
The West Moreton Residential aged care Acute Support Service (RASS) partners with Residential Aged Care Homes (RACHs) to offer alternative care pathways for people who reside in RACHs. This is achieved through:
- Emergency Department (ED) avoidance through ED and hospital substitutive care
- Exploring goals of care with patients and their families to avoid unnecessary tests, procedures, and hospital admission
- Improving transitions of care between the RACH and hospital when hospital level care is required
- Partnering with ED and hospital alternative services
- Improving transitions back to the RACH through follow-up on hospital discharge.
Service delivery predominantly occurs in its patient's primary place of residence, the 16 RACH in the district. After hours, support is offered via the QAS hub, and under the governance of QAS medical officers, some reviews are ‘parked' for review by the RASS service the next morning.
What we're looking for
Our ideal candidate will be someone who can demonstrate the following:
- Experience and demonstrated achievement in similar roles.
- Well-developed interpersonal, negotiation, problem solving, oral and written communication skills.
- Ability to prioritise and coordinate activities in a team environment to meet deadlines and to be flexible in a changing work environment.
- Proven ability in the use of Microsoft Office Programs and the ability to acquire knowledge of relevant software packages (ieMR, ESM, myHR, Medi Records, HBCIS)

- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
