The THHS Single Assessment Organisation is a multi-disciplinary team providing assessment services to older Queenslanders in the community and the hospital setting. The assessment system aims to simplify and improve access to aged care services. We offer attractive remuneration packages and generous leave entitlements.

• Salary range: $128,169 to $137,918 p.a. (HP4)
• 4 to 6 weeks annual leave with 14%-17.5% loading
• 12.75% employer contribution to superannuation
• Plus Salary Sacrificing options may be available

About your opportunity:

Provide clinical and non-clinical home-based face to face assessments of clients referred via the My Aged Care Gateway using the Integrated Assessment Tool (IAT).  Under the Single Aged Care Assessment System, Aged Care Assessors work with clients and their representative/s to establish goals specific to the client's needs and develop appropriate strategies and actions, including referrals to aged care and non-aged care services.

Requirements:

• Mandatory qualifications and proof of registration (if applicable).
• Travel is a requirement of this role.
• Possession of class C drivers' licence.
• This is a Vaccine Preventable Disease (VPD) risk role.
• Probation period may apply.
• Pre-employment checks will be conducted.
