About the opportunity
The position is responsible for ensuring the implementation of individual assessment, diagnosis, treatment, follow up, multidisciplinary consultation, liaison with referring agents, discharge planning and evaluation of care using appropriate outcome and data measurement systems.The Community Care Team provides a range of services to consumers in the short, medium and long term. These include: Individual treatment programs, family interventions, individual and group therapy, psychology services, psychiatric outpatient clinics and case management. The Community Mental Health Service provides community-based assessment and treatment using case management in the context of a multi-disciplinary team, for people with moderate to severe mental health problems.

- Salary range: $4138.10 - $4431.10 p.f. (NG6-1)
- 4 weeks annual leave with 17.5% loading

Requirements:
- Qualification: Bachelor of Nursing or equivalent certification relevant to the position is mandatory.
- Highly Desirable: Possession of or working towards a relevant post-registration and/or tertiary qualifications in mental health nursing.
- Highly Desirable: Eligibility for appointment as an Authorised Mental Health Practitioner (AMHP) under the Mental Health Act 2016.
- Driver Licence: This position requires the applicant to possess an appropriate license to operate a Class C (car) motor vehicle. Verification of license must be provided prior to commencement of duty.
- This is a VPD risk role.
